Program architecture

Five programs. One continuum of care.

Each program has a distinct purpose and is designed to reinforce the others. That protects focus while addressing diabetes as people actually live it.

01 · Find it early

GlucoScreen

Many people live with diabetes or prediabetes without knowing it. GlucoScreen brings screening closer to communities and connects people to education and care before complications develop.

Primary outcomes: earlier detection, fewer undiagnosed cases, stronger community awareness, and clear pathways into care.

02 · Manage it effectively

GlucoAccess

Diagnosis is only the beginning. GlucoAccess helps people living with diabetes obtain glucose monitors, testing strips, lancets, education, and follow-up so they can understand and manage their blood sugar.

Primary outcomes: better access to tools, stronger self-monitoring, more confidence, and fewer preventable complications.

  • Distribution of glucose monitors and testing supplies
  • Training on how to use monitors and interpret readings
  • Self-management education sessions
  • Follow-up through community health workers
  • Partnerships with device companies and health providers

03 · Address food barriers

GlucoNutrition Access

Many people are told to eat healthier without the means to do so. This program pairs education with practical food access because diabetes management is also a food security issue.

Primary outcomes: more realistic healthy eating, stronger nutrition knowledge, fewer food-related barriers, and support for local vendors.

  • Culturally relevant nutrition education based on local foods
  • Healthy food vouchers for eligible participants
  • Partnerships with market women and local vendors
  • Meal planning workshops for families on limited incomes
  • Cooking demonstrations using familiar ingredients

04 · Educate and move

GlucoCommunity

This is where education, fitness, and healthy lifestyles live. Movement is treated as a core prevention strategy, not a separate organization. Weekly walking groups, school wellness days, workplace partnerships, and ambassadors make prevention social.

Signature event: the annual GlucoHealthAfrica Walk for Life — an awareness and fundraising gathering that launches during the Nigeria pilot and can travel with the organization.

05 · Change the system

GlucoAdvocate

Community programs produce evidence. GlucoAdvocate uses that evidence to press for affordability, stronger primary healthcare integration, public investment in prevention, and improved access to supplies.

Primary outcomes: more attention to prevention, stronger policy conversations, and communities as voices in systems change.
